The Clinical Picture

The Impact of Weight Loss: The face relies on deep fat pads to provide its youthful curve and structure. Rapid weight loss (including that from GLP-1 medication like Ozempic/Wegovy or Mounjaro) or the natural ageing process leads to the atrophy of these deep pads, particularly in the temples and lateral cheeks. This loss of subcutaneous volume results in a gaunt, hollow appearance and contributes to skin laxity, as the skin no longer has a scaffold to drape over.

There is also direct effect of these medications on the skin (its collagen structure and elastin content). A large component of the skins barrier , ie the stratum corneum is lipids (fats).

We distinguish between "volume replacement" and "skin tightening," though many patients need both. Where deep fat pads are lost, they must be replaced structurally to support the face. Where the skin has become loose due to rapid deflation, collagen synthesis must be stimulated to tighten the envelope. We avoid the "pillow face" look by placing volume deep on the bone, rather than superficially in the cheek.
