The Clinical Picture

Hyperpigmentation is an umberella term for medical disorders where the melanocytes (pigment-producing cells) become hyperactive. This is often driven by hormonal fluctuations (Melasma/Chloasma) or inflammation (Post-Inflammatory Hyperpigmentation). It can also be driven by sun damage.

Melasma, in particular, is a chronic condition characterised by symmetrical, blotchy patches on the forehead and cheeks. Unlike simple sun spots, it has a vascular and inflammatory component, making it difficult to treat with standard lasers which can sometimes worsen the condition due to heat.

As GPs, we approach pigmentation as a medical condition requiring long-term management rather than a "quick fix." We provide supportive medical consultations to diagnose the specific type of pigmentation you have- this is essential for the correct management. From there, we manage the condition using prescription-strength agents that inhibit pigment production at the source.

Medical Skin Consultation:

A diagnostic appointment to confirm if your pigmentation is hormonal (Melasma) or UV-induced, as the protocols differ significantly.

Prescription Skincare.

We prescribe custom-compounded creams containing Tretinoin, Hydroquinone, or Azelaic Acid. These are the gold standard for suppressing pigment production safely.

Polynucleotides:

An injectable therapy that reduces oxidative stress and inflammation in the dermis. This tends to be used once the melanocytes are more stable.

Treating Melasma requires strict sun protection. We educate all patients on the importance of SPF 50+ to prevent "rebound pigmentation." We often need to think about profection from the visible light spectum using tinted SPFs. Prescription agents are monitored closely; we review you regularly to ensure your skin barrier remains healthy and to adjust the strength of your active ingredients as needed.
